10/24/2025 For “forms code for Monthly meeting,” a “form” can refer to a digital feedback or status form, or a template for meeting agendas and minutes. While there is no universal “forms code,” you can create your own for platforms like Google Forms or use pre-built templates for Microsoft Word and other tools.

Creating a monthly meeting form using Google Forms

If you want to gather input or feedback from meeting participants before or after a meeting, you can create a custom form. The code is generated automatically by Google Forms; you simply copy and paste it to embed the form on a website.

Example questions for a pre-meeting status form

  • Name (Short answer)
  • What is your primary focus this month? (Paragraph)
  • What are your key accomplishments since the last meeting? (Paragraph)
  • What challenges or blockers are you currently facing? (Paragraph)
  • What is one thing you would like to discuss at the meeting? (Paragraph)

Example questions for a post-meeting feedback form

  • Overall, how effective was this meeting? (Linear scale: 1-5)
  • Were the agenda items relevant to the team? (Multiple choice: Yes, No, Some were)
  • Did the meeting stay on track and on time? (Multiple choice: Yes, Mostly, No)
  • What was the most valuable part of the meeting? (Paragraph)
  • What could be improved for the next meeting? (Paragraph)

Using a template for your meeting agenda and minutes

A standard practice for recurring meetings is to use a consistent template. The “code” in this case is the formatted document itself, which provides a structured record of each meeting.
